I ended up buying a CT-8026 because that was the model number on the existing one. I believe my monitor is a Wells Gardner though. All the parts, boards etc. have WGxxxx part numbers on them.

It looks like this flyback has two pin-like protrusions that the original flyback does not. What do you do with them? Other than that, it looks like the holes will line up. Thanks.

Not yet. I'll be making a call to LA Slots shortly though. I think I've determined in talking with some other folks that it's the flyback transformer. Wish me luck.
