Check the adjustment of the left outlane switch. I would guess that it's not always activating. Always check these using a ball. If you use your finger, you can press down further than a ball can. To fix it, you can bend the wire a little bit, but I have had better success with loosening the screws and changing the angle of the switch just a little. If the switch still isn't reliable, replace it. I use the good Cherry switches that Pinball Life sells:
