Here is the topper I have been working on. Sorry about the quality of the pics. I'll try and get some better photo's including blacklight shots when I find the time.

The place I got the materials at is called Tap Plastics. I'm not sure if they are nation wide or just the west coast. The products are pretty standard for any store that sells plastics.
